Your physician is sending your specimen to Logan Laboratories for processing. As an independent laboratory, Logan's testing is designed to assist your physician in managing your medications to ensure you are receiving optimal therapy and dosages to treat your pain symptoms. This provides you with the safest and most effective treatment.

Logan Laboratories will verify your in-network benefits depending on your insurance and based on that verification, we will make the proper adjustments and bill you for your unmet in-network, copay, co-insurance or deductible.

You will receive a statement that will indicate patient responsibility within approximately 4-6 weeks. Patients can call the number at the bottom of the billing statement or follow the website link printed on their statement for online payment options.

We will waive patients' commercial insurance out-of-network financial responsibilities and instead bill them as if we were an "in-network" facility. We are contracted with Medicare and various State Medicaid & Workers Compensation plans. Patients are only responsible for their in-network, unmet deductible plus any co-pay or co-insurance. Patients will receive an Explanation of Benefits (EOB) from their insurance company, This is not a bill. Patients should call us at  (844) 774-5853 before making any payment to Logan Laboratories.
